LAWRENCE — Kansas University officials say a lab accident in Malott Hall in early November was caused by a graduate student who failed to follow proper procedures in handling a toxic chemical. The student was injured and hospitalized for three days after he ingested sodium azide. KU said the student apologized to investigators and his instructors afterwards.
